Incident Reports

Submit and review ski patrol incident reports. All reports are confidential and for internal patrol use only.

Incident reports are confidential patrol records. This section is restricted to active Afton Alps Ski Patrol members.

Reporting Guidelines

Submit promptly
Reports should be filed the same day as the incident. Delayed reports may be flagged by the patrol director.
Be accurate and objective
Record only what you directly observed. Avoid speculation about cause or fault. Use the patient's own words when documenting complaints.
Confidentiality
Incident reports are internal patrol documents. Do not share report details with guests, media, or third parties. Direct all external inquiries to the patrol director.
Follow-up required
For serious incidents or EMS activations, a follow-up debrief with the patrol director is required within 48 hours.

Complete this form as soon as possible after an incident — ideally before leaving the hill. All fields marked required must be filled in before submission.
